JOB SUMMARY:
The Community Health Assistant is an instrumental member of a multidisciplinary team, working within the community providing outreach to patients with complex medical and social needs. The Community Health Assistant acts as a resource to assist patients in navigating the health care system. Under the direction of the Case Manager and the LSW, serves to facilitate linkage to community resources that will enable the patient to obtain optimal health outcomes and remain safe in the community.
MAJOR DUTIES AND RESPONSIBILITIES:
1. Assists in the outreach of patients identified as medically complex.
2. Collaborates with the multidisciplinary team in the reinforcement of the care plan.
3. Engages patients by way of home visit, hospital visit and accompanying patients to Providers appointments as needed.
4. Facilitates adherence to treatment plan by early identification and resolution of gaps in care.
5. Communicates all concerns to clinical team and manager as necessary, seeking support and guidance proactively.
6. Participates in multidisciplinary team conference in order to maximize the treatment plan.
7. Provides reinforcement of the treatment plan under the direction of the Case Manager and/or Licensed Social Worker.
8. Appointment scheduling and clerical duties as needed.
9. Builds relationships within the community in order to identify resources that are available to the patients i.e.: DME vendors, Home Health Agencies, Area Aging Office etc.
10. Performs all other duties as assigned by program manager or designee.

Demonstrates proficiency in basic computer skills, including accurate keyboard entry of data into relevant computer based databases and spreadsheets
Attention to detail, and ability to be flexible in performing a variety of tasks
Demonstrated ability to complete projects within designated timelines and the ability to prioritize duties is required
Demonstrates ability to use basic office equipment, computer, fax machine, copier, calculator, etc.
EDUCATION AND/OR EXPERIENCE:
High School Diploma required.
Three years of experience in community based outreach or demonstrated progressive advancement in responsibilities in healthcare setting such as clinic, hospital, etc. required.
Valid PA Driver?s License
WORKING CONDITIONS/PHYSICAL DEMANDS:
Varies settings including but not limited to office, patients home, and hospital.
Travel required, personal vehicle with mileage reimbursement
Day shift with some evening hours required

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
